Athlete Athletic Clearance Information
In order to compete in athletics at King Drew you must complete the following:
1) Athletic Physical - Have a physical conducted by a healthcare professional (MD, PA, DO, NP) on file.
Physical forms - Below are the required forms for sports in LAUSD:
⚠️ Warning: Before your turn in your physical check for common mistakes:
Pre-Participation Physical examination form
a) Pen only - Only physicals that are filled out in blue or black pen will be accepted. No pencil or colored pens!
b) Yes Answers - All "Yes" answers must be explained. Heart issues and/or a family history of heart issues may require additional documentation from your doctor.
c) Allergies and Asthma - If you have a specific allergy or asthma you will be required to turn in additional documentation which is included in the physical packet.
d) Competed form - Make sure you and your parent complete the whole form. If you are missing information (name, sport, signature etc.) your physical will be returned to you for corrections.
Physical Examination Form
a) Stamp - Make sure the Physical Examination form includes the clinic's stamp.

Home Campus Registration - Fill out the required athletic department forms on
HomeCampus.com. These forms include: The forms included are: Emergency Card, Parent Consent Form, Insurance Certificate Risk and Consent, Code of Conduct, Steroid Prohibition, Concussion Form, Sudden, Cardiac Arrest Form, Trip Permission Slip, Anti Bully/Hazing Contract, Publicity, Authorization and Release Form, Out of Season Liability Advisory, and Athletic Uniform Contract.

GPA requirement - All students must have a 2.0 or above GPA.
Failure to adhere to clearance deadlines will result in a delay in the clearance process. No athlete is allowed to practice unless they are cleared. Your coach will inform you if you are cleared or not. Check with your coaches for clearance date information.
