Note: If a coach is transporting a student in their personal vehicle to an athletic event the school must have the Transportation Waiver and the Private Automobile Driver Certification of Liability insurance (both of these are in the Private Auto Bulletin file below) forms on file. In addition, each student must have a signed Field Trip Parent Permission Slip.
